Online Screening Questionnaire
(Updated for Spring 2022) We will not be emailing you the COVID-19 Screening Questionnaire each week. Only if you are unable to attend class because of symptoms, testing, or quarantine, you may fill out the questionnaire on our website (click button below) to request for your teacher to set up a live Zoom link of class to participate virtually, and we will try our very best to accommodate this request. Requests must be made by 3:00pm the day of class so teachers have time to prepare. Thank you!

When NOT to come to class:
--Students must be “fever-free”, for 24 hours without the use of fever-reducing medications before returning to dance class.
When NOT to come to class:
- Have symptoms of infectious illness or certain symptoms of COVID-19, particularly:
- Fever (temperature 100.4 ºF or higher)
- Sore throat
- New uncontrolled cough that causes difficulty breathing
- Recent loss of taste or smell
- Diarrhea, vomiting, or stomachache
- New onset of severe headache
- Currently waiting on the results of a COVID-19 test, had a positive test , been exposed, or worried about being sick with COVID-19.

For the safety of our children and families...
- (Updated March, 27, 2022) Face masks are OPTIONAL for all
- If you are not up to date on COVID vaccines (especially if at higher risk) and waiting in the building/dance rooms, please maintain a social distance of at least 6 feet from others not in your family/immediate group. Efforts will be made to social distance dancers during class when possible.
- Christ Church (classes in the Sanctuary): After bringing your dancer to class, you may choose to:
- wait in the lobby during class (there is a window into the dance room),
- or return to your car to wait / leave and come back for pick up (please return on time to be ready to pick up your dancer).
- First Baptist Little Rock (classes in the Fellowship Hall): enter Door 1 [See email for instructions and Door Code]. After bringing your dancer to class, you may choose to:
- wait quietly (as to not distract the dancers) in the back of the Fellowship Hall (chairs may be available, or you are welcome to sit on the floor),
- or return to your car to wait / leave and come back for pick up (please return on time to be ready to pick up your dancer).
- First Baptist Little Rock (classes in the Racquetball Room): enter Door 6 [See email for instructions]. After bringing your dancer to class, you may choose to:
- wait in the lobby,
- or return to your car to wait / leave and come back for pick up (please return on time to be ready to pick up your dancer).
- Hand sanitizer will be accessible at the door of the dance classes. Restrooms are available for handwashing. Facilities are cleaned by the facility staff.
Dancers, please remember...
- Please bring a water bottle clearly labeled with your name, as water fountains may not be accessible. Food is not allowed in class.
- Please avoid personal contact, including hugs, hand shaking, and high fives (smiles, air hugs, waves, and air fives are always welcome!)
We will continue to look to the CDC and AR Department of Health for guidance on how to offer classes safely. New Creation reserves the right to adjust classes or policies at any time due to COVID-19 regulations/guidance. You will be notified by email of any changes.
