I give New Creation Dance Company and its members permission to take pictures/videos of myself/my dependent during New Creation dance activities (including dance classes, workshops, fundraisers, rehearsals, and performances), and also to use those pictures/videos for promotional purposes on their website, social media pages, fliers, or for any other form of advertising in the future. *

I therefore agree to assume all such risks, and exempt, release, and indemnify New Creation Dance Company and its members from any liability loss or injury to me/my dependent or my (dependent’s) personal property which may occur due to any cause in relation to participation in New Creation activities (including dance classes, workshops, fundraisers, rehearsals, and performances). *

Registration Fee:

$20 per student due by August 31st
Late Registration after August 31st: $25 per student (due by student's first class)

NCDK T-shirt:
- $15 added to Registration Fee (if student does not already have one)
- Light blue; New Creation Dance Kids logo on front; 2 Corinthians 5:17 on back (to be worn for Fall fundraiser performance and other outreach events not requiring a costume)
